Reable - Rehab to Enable

ReABLE is a multidisciplinary, community based rehabilitation service. We are a TEAM that offers care/support/advice/assessments/rehabilitation within your own home. The team enables you to STAY AT HOME following a fall, operation or stroke or if you are struggling to cope because you are getting older or have been diagnosed with a long term condition or terminal illness. ReABLE is made up of a physiotherapist and carers and works closely with a team of other healthcare professionals so that all your therapy is coming from one team who are working together to maximise your independence!
